## When to use it

Use when customer risk is discovered too late, signals are scattered, or teams disagree on account health. Do not use it to create a dashboard without playbooks or validation.

## Inputs to collect

## Method

1. Define the outcome the score should predict and the lead time required for action.
2. Separate leading signals, lagging outcomes, customer-reported evidence, and internal activity.
3. Weight signals by predictive value, freshness, segment fit, and actionability; define missing-data behavior.
4. Create health tiers with named interventions, owners, SLA, and escalation.
5. Backtest, inspect false positives/negatives, calibrate thresholds, and review drift.

## Output

Produce a health model, signal dictionary, weighting and freshness rules, tier definitions, intervention playbooks, dashboard spec, validation report, and review cadence.

## Verification and failure modes
